Forms for Supported Lives staff

Timesheet

Sessional staff must complete this form to the pay date and submit to office by the relevant date (see pay chart below). Senior Support Workers and Community Support Workers must only fill in this form if they have worked a Bank Holiday or agreed overtime.

To guarantee payment, forms must be received in office within specified date. Forms received after the cut-off date will not be processed until the following month

Expenses claim form

Make sure that you print off both sheets of this form and put your name on both sheets. Remember to put all your receipts for non-mileage expenses in with the form.

Mileage recording sheet

All mileage done in your own car that will be charged to clients must be recorded on one of these sheets.

Pay chart

A list of the pay dates for 2008-9 with the deadlines for receipt of hours worked and expenses forms.

Holiday request form

Remember to get your holiday request in at least four weeks before you want to take a holiday.
