7. Care plans
Referrals for support will only be accepted if there is a full care assessment in which support needs are identified and agreed.
- The care assessment must list the tasks with which assistance is required - for example washing, dressing, medication - and the degree of support needed.
- The detail of the assessment may be subject to negotiation following an initial visit/risk assessment.
- Community Support Workers will carry out only those tasks agreed in the care assessment, but must inform their Team Leader if they become aware of any gaps, changes in need or over-provision. It may be appropriate to review the care assessment at that stage.
