Community based (domiciliary services) are necessary to enable an individual who meets the eligibility criteria to remain at home with a reasonable level of health, hygiene, safety and comfort, and delivered in such a way as to enhance their quality of life. The service may be needed due to a permanent or temporary inability to manage their own care, because of frailty in advancing age, disability, physical or mental ill health, changes within the family causing the need for extra help, or the lack of necessary self-caring skills. The package of care may include personal care, rehabilitation, community meals, shopping, collecting pensions and allowances, or other services identified in the care plan.
